Local Priorities
The Integrated Health Service Plan (IHSP) describes priorities the Champlain LHIN will focus on over the next three years. It sets out a direction for the LHIN to reach its vision and meet its mandate of improving the local health system at a local level while working toward provincial priorities. The plan explains where we will place our attention and resources, what results we plan to achieve, how we plan to achieve them and how we will know we have made a difference.
Key Result Areas
The Champlain LHIN will work with our provincial and local partners to continue to build on our successes and address our local health care system challenges. Over the next three years, we wish to see results in six important areas for Champlain residents:
• More people involved in planning their health services
• More people receive quality, evidence-based care
• More people with mental health conditions and addictions have access to services
• More seniors are cared for in their communities
• More people with complex health conditions are able to manage their conditions
• More people at end-of-life, families and caregivers receive palliative care supports in their setting of choice.
Strategies for an Integrated System
We will achieve our six key results by putting our efforts and resources toward making changes in three important areas:
• Build a strong foundation of integrated primary, home and community care
• Improve coordination and transitions of care
• Increase coordination and integration of services among hospitals.

How was the IHSP 2013-16 developed?
In early 2012, the Ontario Ministry of Health and Long-Term Care published Ontario’s Action Plan for Health Care. The 14 LHINs across the province are working together and with the Ministry of Health and Long-Term Care to advance this plan for health system improvements.
The plan was also shaped by the information we gathered from the many engagement activities we conducted over the years. We listened to what people and health service providers in our communities have told us about their health needs and challenges. For example, people told us we need to:
• Improve and simplify access to services
• Establish ways for providers of care to share information
• Create a more organized, “user-friendly” health system; and
• Ensure quality services are available in all parts of the region.
Provincial Priorities
IHSP 2013-16 aligns with these provincial priorities. Examples include:
• Faster access to family and community care
• Quality, evidence-based care at the right time and as close to home as possible
• Better coordinated and integrated care
• More supports so seniors can stay healthy and at home longer.
Local Priorities
How do we put the “local” in health care? Over the years, the LHIN and our partners have worked hard to make health system improvements in our cities, rural communities, with our Francophone population and our Aboriginal Peoples. IHSP 2013-16 builds on local improvements already made or in progress.
To better understand our regional health system, the Champlain LHIN has looked at the health profile of our residents, how they use health services, and information about how those services are performing. These vignettes and person-centred evidence illustrate how this information has been used to help shape our proposed plan.
