French-Language Health Services Delivery

  • For All Providers: Reporting Requirements

One obligation in the Service Accountability Agreement (SAA) is that each Champlain LHIN-funded Provider must submit annual reports on its delivery of French Language Health Services (FLHS).

Providers will need to submit these reports using OZi (external site). For technical support, contact OZi support staff (info@oziweb.ca, or toll-free 1.866.694.9320 option 1).

For guidance and support on FLHS accountability reporting requirements, please contact Lise Racicot (lise.racicot@lhins.on.ca).

  • For Providers that are Non-identified

This document assists those Champlain LHIN-funded Providers that are not identified to provide services in French.

It helps Providers to fulfill their new responsibilities in their SAA, and in the Ministry’s Guide to Requirements and Obligations Relating to French Language Health Services (FLHS), mainly through the development of an FLSH plan.

  • Reporting on French-Language Health Services Delivery
