The Champlain LHIN is committed to involving more people in planning their health services. As part of this, and in response to a need identified by health care providers in our region, the LHIN has partnered with Pinecrest Queensway Community Health Centre to create a collection of engagement resources.

Specifically, this collection supports efforts to engage patients/clients, caregivers and families* in the design, planning, coordination of health care systems and services.

The Champlain LHIN and Pinecrest-Queensway Community Health Centre would like to acknowledge the support of the Canadian Foundation for Healthcare Improvement in the production of this resource, including the contribution of materials and content.

* Throughout this guide the term "family" refers to family as defined by the patient/client.


This resource collection is intended to support health care providers, planners, and networks that want to get started in patient/client, caregiver and family engagement.

Ranging from the strategic to the tactical, you will find self-assessments, frameworks, approaches, forms, templates, and more. We also included a section on engaging with communities and specific populations to support broader engagement strategies.


The resources in this collection represent a range of perspectives and approaches, and were selected from regional, provincial, Canadian and international sources. They were selected because the materials are health-care related, and as much as possible, available in English and French.
